Filing a Claim

City Claim Forms are available:

• From the City Clerk’s Office: 2263 Santa Clara Avenue, room 380;
• From the City Attorney's Office: 2263 Santa Clara Avenue, room 280; or
• From the link below.

Claims must be filed with the City Clerk's office in a timely manner as provided by law.  All claims are overseen by the City Risk Manager.
