STEPS TO APPLY FOR A NEW PASSPORT
(Passport RENEWALS must be mailed directly to the National Passport Processing Center)
Fill Out An Application
Passport Application Form DS-11
- Do not sign the application until instructed to do so by the acceptance agent.
- You must provide a Social Security Number (SSN) in accordance with 22 U.S.C. 2714a.
- You may request a larger passport book with 52 pages, at no additional cost, by checking the 'Large Book' box at the top of the DS-11.
Provide Evidence of U.S. Citizenship
You must submit one of the following. Photocopies and notarized copies are not acceptable.
- U.S. birth certificate that meets the following requirements:
- Issued by the city, county, or state of birth
- Lists your full name, date of birth, and place of birth
- Lists your parent(s)' full names
- Has the date filed with registrar's office (must be within one year of birth)
- Has the registrar's signature
- Has the seal of the issuing authority
- Fully-valid, undamaged U.S. passport (may be expired)
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Naturalization
- Certificate of Citizenship

Present Identification
You must present one of the following identification documents (IDs) to the acceptance agent. The ID must readily identify you.
In-state, fully-valid driver's license
Fully-valid, undamaged U.S. passport (may be expired)
Certificate of Naturalization or Citizenship
Government employee ID (city, county, state or federal)
U.S. military ID or military dependent ID
Valid foreign passport
Matricula Consular (Mexican Consular Identification, commonly used by the parent of a U.S. citizen child applicant)

If you present an out-of-state ID, you must present an additional ID.

Visit City Hall to complete your paperwork
You must submit your: completed application, including citizenship evidence, ID, photo, and fees, in person to the Alameda City Clerks Office located at 2263 Santa Clara Avenue #380 Alameda, CA. 94501.
Fees to the Department of State are payable by check or money order only.
Passport hours are Monday through Thursday from 9:00 a.m. to 3:00 p.m.

Passport Photo Services
We offer same day passport photos for a fee of $25. If you are providing your own photo please follow these guidelines.

FREQUENTLY ASKED QUESTIONS
Processing times can vary depending on the time of year, and occasional unforeseen circumstances such as natural disasters. During busier times, such as the summer travel season, we encourage customers to expedite their applications if traveling in less than 6 weeks. See Application Processing Times for more information.

Yes, you will receive the old, cancelled passport. It may be sent separately from your new passport. It is a good idea to keep your old passport in a safe place as it is considered proof of your U.S. citizenship.
No, you cannot. Applicants in need of additional pages in their valid passports must obtain a new passport by mail. Applicants within the United States may choose a 28-page or 52-page book.
